Psychological Assessment for child, Psychological Evaluation

Accepted forms of payment include cash, check, & most all U.S.-based credit and debit cards. We also accept FSA and HSA cards for payment.

Psychological Evaluation services are billed directly to the family. The total fee for a Psychological Evaluation is based on the number and type of tests included in the assessment battery. An estimated fee for the evaluation may be discussed during your free phone consultation. A personalized fee schedule may also be provided upon request following the Intake Interview. Dr. Kealoha's out-of-pocket fee for service is $300 for the Intake Interview and per hour of Psychological Evaluation service. Payment can be made via cash, check, or debit, credit, FSA, or HSA card payment. For additional information, please refer to our Psychological Evaluation Process description on the FAQ page.

Seeking Insurance Reimbursement for Services?

We've Got You Covered.

Many health insurance plans for which Kealoha Psychology, Inc. is out-of-network will reimburse families directly for a portion of the service fee (typically between 40% and 70%). After the service has been completed and all services have been paid, Kealoha Psychology, Inc. will send you a special invoice called a "superbill" which will detail the services rendered, applicable diagnoses, and payments received. If you or your child is covered by a health insurance plan that includes out-of-network benefits, you can submit the superbill to your health insurance provider to seek reimbursement for a portion of the cost. To verify your out-of-network benefits, we encourage you to contact your health insurance company. *Please also see our tips below for seeking reimbursement from your health insurance and the procedural codes billed for Psychological Testing.

*TIPS FOR SEEKING OUT-OF-NETWORK REIMBURSEMENT:

Reimbursement

STEP 1: Contact your health insurance provider via phone. The member services phone number can typically be located on the back of your health insurance card.

STEP 2: Ask if you have “out-of-network” benefits for mental health services. Specifically, for a Psychological Evaluation and Testing services completed by a licensed Psychologist.

STEP 3: Ask if you have a deductible before your coverage kicks in. You can ask the representative if you have a deductible for out-of-network mental health services. If so, ask what the remaining amount is that you would owe before your mental health plan starts to reimburse you for any fees that you pay out-of-pocket.

STEP 4: Find out how much your health insurance plan will reimburse (find the CPT CODES we bill here). You may ask the representative the maximum amount your plan will reimburse for Psychological Testing. The specific mental health procedural codes for psychological testing are 96136, 96137, 96130, and 96131. The intake appointment is billed using procedural code 90791.
